Because of Dr. Leigh's extensive experience with augmentation mammoplasty, revisional augmentation is a significant portion of his practice. With time, even the best augmentation mammoplasty may be affected by further tissue sag or formation of capsular contracture that requires further surgery. Other reasons for revisions may be dissatisfaction with the original surgery resulting in appearance or feel problems, or may simply be a desire to be larger or smaller. To best correct these problems, a detailed history is needed with information about the placement of the implant above or below the muscle, the type of implant, time since surgery and any significant events that may have lead up to problems. These problems could include trauma, infection, or pregnancy. Most importantly, the patient's goals must be discussed to arrive at a realistic treatment plan based on physical exam findings. Revisional surgery is a specialized area of augmentation surgery and may require procedures not typically done with an original augmentation such as use of drains, massage techniques, prolonged breast fold support, or even use of oral medications to try and reduce undesired scar formations.
